Faculty Notification Letter

For current students who are satisfied with their current accommodation plan, please contact the Office of Disability Services using this form to request your faculty notification letter. This notification letter will be effective only for the current academic year (Fall – Summer), and is effective for all terms and modules. As before, accommodations cannot go “into effect” until you have notified your faculty.

Process for Requesting Additional Accommodations

The Office of Disability Services recognizes that accommodation needs may be fluid, or that certain disabilities may expand. Students who are experiencing concerns, that are not currently provided for with their current accommodations, are welcome to request additional accommodations. This process would begin by completing the Request for Additional Accommodations form. When completing the form, the student should upload all supporting documentation that speaks to the new concern and suggested accommodations.

Once the Request for Additional Accommodations form has been submitted, students should contact the Office of Disability Services to request an updated intake meeting. In this meeting, the student should be prepared to discuss their concerns and challenges, as well as why these may not be helped by current accommodations.

The Office of Disability Services will review the application, along with information gathered in the intake meeting, and prescribe additional accommodations as needed. If approved for additional accommodations, students will receive an updated Faculty Notification Letter to use for that academic year.
