Course: Case Management (PY-536-1)
This course examins concepts and principles of case management practice with special populations such as older adults, children andfamilies, persons with serious and persistent mental illness, substance abuse, and persons with chronic disease and those facing end of life decisions, The core functions of case management practive in a range of settings are addressed in relationship to issues of diversity, Vulnerability, and empowerment.
